Vladislav Volkov

Vladislav Volkov was a Soviet cosmonaut and engineer who flew on Soyuz 7 in 1969 and later on Soyuz 11 in June 1971, the mission that carried the first crew to live aboard the worlds first space station, Salyut 1. During reentry, a valve on the Soyuz 11 capsule failed and depressurized the cabin, and Volkov died along with his two crewmates, Georgy Dobrovolsky and Viktor Patsayev, the only human deaths to occur in space rather than in Earths atmosphere or at the ground.
